The sergeant said alcohol and, possibly, other intoxicants may have been factors in the crash, which remains under investigation.

Investigators remain on the scene, and as of 9:10 a.m., the two left lanes remain closed in the area, police said.

Tinlin says such manhole covers weigh more than 200 pounds.

State police are investigating how the manhole cover became dislodged.

The manhole cover that killed the woman covers a storm drainage system and was last inspected in June 2014, Tinlin said.

“The State Police investigation has determined that the victim’s vehicle in this crash was struck by a metal manhole cover that become dislodged and airborne”. There was no indication the cover had been removed recently, he said.

Massachusetts State Police say a manhole cover crashed through a windshield on a major Boston highway during the morning commute, killing a driver. They added that it appears the vehicle continued to travel in the left lane of Route 93 southbound until it hit a wall on the left shoulder and stopped.
