According to the image of form, a five year old kid tried to copy his mum’s signature on the form in parent signature’s column.
The image of form was uploaded in the social media, Imgur, by his mother with caption “My 5-year-old’s attempt to forge my signature. Nailed it”.

However, we don’t know yet exactly why this kid had tried to copy his mum’s signature in the form but we only assume that there might be some kind problem in school that he didn’t wanted to tell his mother.
